Probably the "4x3 cutout" is done by the local channel providing the feed to Verizon.  Verizon has chosen to mostly do the letterbox for SD of channels that are provided as 16x9 HD nationally.

Possibly a sticking key on the remote.  Certainly it is not commonly reported that Verizon DVR's are doing such recordings on their own.

Yes. This is exactly what I do.   I have a Tivo Roamio (you can also use the newer Tivo Bolt).  You need the version which is CableCard / Digital TV Ready, not the Over-the-air version they also sell....

I am able to use a video capture program to copy (at playback speed) the video thru composite (RCA) cable to my computer.  It creates a mpg file that is about 1mg per second of play.

A number of steps are necessary.  Ideally first test on the same network as your STB is located on.  You must find the screen in your VMS1100 to enable the sharing and  then share to your Verizon app....
